Participant Information & Media Release

By submitting this form and participating in the Tax Pro Photoshoot, I hereby grant United eFile LLC, Aarius Dumas Productions LLC and its affiliated brands, partners, and clients (collectively, the “Producer”) the irrevocable right and permission to photograph, film, record, and otherwise capture my likeness, image, voice, name, professional title, and tax office affiliation during the photoshoot session.

I understand and agree that such photographs, video recordings, and related materials (“Media”) may be used, edited, duplicated, distributed, and published in whole or in part, in any medium now known or later developed, including but not limited to websites, social media, digital advertising, print materials, presentations, marketing collateral, training materials, and promotional campaigns, without further notice or compensation.

I acknowledge that:

• My participation is voluntary.
• I waive any right to inspect or approve the finished Media or its use.
• I release and hold harmless the Producer from any claims, demands, or liabilities arising out of or related to the use of the Media, including claims for defamation, invasion of privacy, or rights of publicity.

I further consent to the collection and use of the information I provide below for:

• Identifying the Media content,
• Associating it with my tax office or professional brand,
• Internal recordkeeping,
• Marketing, promotional, and informational purposes related to the Producer’s business activities.

I certify that I am at least 18 years of age and have the legal authority to grant this consent.
